Parent/Guardian Permission Form
The Parent/Guardian Permission form must be submitted once for each student, beginning in Fall 2007. The permission form is valid throughout the student’s period of eligibility for the Early College Progam.
Submission Information
Submit Course Request, Signature and Fee form and payment with signature of student, along with Parent/Guardian Permission form.
- The Parent/Guardian Permission form is to be submitted once (beginning Fall 2007) for an individual student.
- The Parent/Guardian Permission form is valid throughout the student's eligibility for the Early College Program.
- Please include a school official’s signature when indicated.
- Students who have reached the age of 18 do not require submission of the Parent/Guardian Permission form.
